DFW Group Member Form

Welcome to the Bill Glass Champions For Life Group Form. If you have a group of teammates that would like to be assigned to the same unit on a Bill Glass prison event, please fill out the form below.

Please note:

1. Make sure the members of your group have registered before you submit this form. If they are not registered when the form is submitted we cannot guarantee they will be assigned to the same unit.
2. Enter the group members one per line. Use first and last legal names.
3. Should you need to add additional members to your group, fill out the form completely but only list the names of the new members.
4. All members of groups larger than 10 people are not guaranteed to be assigned to the same unit. If a unit cannot handle a group of 10 or more, the group will be split into smaller groups.
5. The deadline for the group form is the day after the deadline for event registration.
